Rather than have a formal order form, I would prefer that you contact us at radioplus@bellsouth.net to determine the current availability of an item of interest. This will also give you an idea of total shipping costs, applicable discounts, and target shipping dates. Note that all items are shipped via USPS Priority Mail to US addresses. Contact us for international shipping costs or alternative shipment methods.
I'd also like to know what kind of equipment you have, and perhaps your area of DX interest, so that I can maybe recommend for or against certain purchases. (For example, few portable radios can be used to good effect with the Quantum Phaser.)

We accept money orders and checks (waiting period is sometimes necessary) for payment. We also accept credit card orders BUT ONLY THROUGH PAYPAL.
